Letters of Recommendation

A very serious and important part of my academic life is providing you with letters to help you to the next stage of your career. But I need some help from you to get the job done efficiently and accurately.

1.Please do not ask me for a letter if I do not know you. It is difficult if not impossible for me to provide you with a good letter if I have had little interaction with you.
2. Be kind and give me four to six weeks to prepare the letter.
3. Once I have agreed to write a letter, deliver the following two sets of materials to me in the form of a single <last_firstname>.pdf file for digital files and a single file folder with your name on it for paper and addressed envelopes. Please put what you can in digital format as it reduces the number of papers I have to carry with me.

a. Your c.v.
b. A statement of purpose or your entrance essay if you have one.
c. A copy of your senior thesis if you have one
d. A page in Word or excel format which lists
I. The places you are applying, the due date for the letter, details about the letter submission (online/paper), website URLs
II. If it is a paper letter note if I send it in or you do.
III. Your complete contact information (cell/e-mail)

e. A transcript with your GPA
f. All of the forms that need to be filled out - with the following information typed in (unless you download the forms, you will have to use a typewriter for this):

Name: Patrick F. Kiser
Occupation: Assistant Professor of Bioengineering
Department: Bioengineering
Mailing Address: University of Utah
BPRB RM 108
20 South 2030 East
Salt Lake City, UT 84102
Phone: (801) 587-3681
Fax: (801) 585-5151

g. Pre-address and stamp the envelopes for mailing. Place a return address in the upper left hand corner that reads:

Patrick F. Kiser
University of Utah
BPRB RM 108
20 South 2030 East
Salt Lake City, UT 84102

4. If you are applying to multiple places, please give me all requests at once. I like to complete all of your letters in one batch.

Thank you for help in this important process.
